- Full-time Job Overview Supports LCCA in completing admissions and discharge documentation, scheduling and completion of services, service planning, and file compliance for children placed with Legacy Foundations Residential Campus, a group home for youth in foster care.
QUALIFICATIONS
A Master's degree or higher from an accredited college or university; or a Bachelor's degree from an accredited college or university in social work or other human services field with one year of documented full-time work experience in a residential child-care operation, or related field of child and family services. Or A Bachelor's degree from an accredited college or university with two years of documented full-time work experience in a residential child-care operation, or related field of child and family services Job Duties Assists LCCA with completion of referral process and gathering of required information at time of placement. Conducts intake meeting with LCCA and referring agency and completes orientation upon acceptance Provide TBRI training and assistance to all staff according to treatment model Completes safety plans as needed and submits to LCCA for approval. Requests/collects information needed at time of placement regarding child's identity, history, medical, conditions, allergies, etc. and provides information to direct care staff. Schedules EPSDT to occur within the required time frame. Ensures that all medication administered is documented in accordance with Minimum Standards. Completes admission assessments or any other evaluation or documentation needed for placement. Schedules all required appointments and recommended services. Maintains record compliance. Provide Aftercare case management and follow Aftercare Policies and procedures Develops, reviews, and updates service plans for children in care as required. Approves any restrictions that will be imposed on a child for more than seven days that have not been reviewed and approved by the service planning team, and any monthly re-evaluations of restrictions that continue more than 30 days. Approves any restrictions to communication and visitation with the child's family that are imposed on a child, but have not been reviewed and approved by the service planning team, including monthly re-evaluations of restrictions that continue for more than 30 days. Approves any restrictions to a particular room for more than 24 hours that are imposed on a child, but have not been reviewed and approved by the service planning team. Serve as the educational liaison and ensure all educational plans are followed and implemented throughout the youth's education and participate in all educational meetings (ARD,CONFRENSE, Child specific meetings) PLSP will be responsible for signing and dating documents to indicate review and approval or disapproval (documents include admission assessments or any other evaluation, initial service plans, updates/reviews, discharge and transfer summaries, restrictions that will be imposed on a child for more than seven days that have not been reviewed and approved by service planning team, restrictions to communicate and visitation with the child's family that are imposed on a child, but have not been reviewed and approved by the service planning team, any restrictions to a particular room or building for more than 24 hours that are imposed on a child, but have not been reviewed and approved by the service planning team. Completes extension forms for children placed with Legacy Foundations. Completes discharge and transfer summaries for children as required. Advocates for appropriate interventions for children. Provides recommendations for crisis intervention decisions as needed. Maintains confidentiality of all agencies, client, and personnel information. Ensures compliance with all governmental laws and regulations, DFPS minimum standards included Chapter 42 of the Human Resources Code, Chapter 745 and any other laws which are relevant to the person's job duties, as well as contract requirements, and Legacy Foundation's policies and procedures. Reports immediately any suspected incident of abuse or neglect or exploitation of a child in care to the Executive Director, LCCA, and the Child Abuse Hotline. Demonstrates competency, prudent judgment, and self-control in the presence of the children and staff and when performing all assigned tasks. Advocates for client services as needed or recommended, Consults with LCCA and Executive Director regularly regarding admissions and discharges. Completes other tasks as required by LCCA or the Executive Director. Behavior or health status must not present a danger to children in care. Meets requirements in Subchapter F or Chapter 745- relating to background checks(Cleared background check and background check requirements must be completed prior to beginning job duties) Provides a TB screening indicating the employee is free of contagious TB for personnel record.
