Advocacy 1201 Kitty Hawk Rd. Universal City, Texas, 78148 United States Outside working hours if other than M-F 8:00 a.m. to 5:00 p.m.: Will include some nights and weekends, Monday - Friday 8-5. Job Summary and Description Provide case management as a best practice ensuring excellence in service and documentation in all phases including screening, assessment, stratifying risk, planning, implementation, follow-up, transitioning, communicating post-transition, and evaluation. Case managers participate as an interdisciplinary team member in both staffing and consultation to ensure the continuum of care for essential needs as well as mental health.
Qualifications Minimum Education and Experience:
Bachelor's degree in social work. Two (2) years of directly related social work experience including case management.
Preferred Education and Experience:
Master's degree in social work or LMSW. Five years of experience in social work.
