Full-Time Case Manager A Better HCS Home Inc. - Waco, TX Position Summary A Better HCS Home Inc. is seeking a compassionate and detail-oriented Full-Time Case Manager to coordinate services for individuals enrolled in the Home and Community-Based Services (HCS) program. This role focuses on supporting individuals with intellectual and developmental disabilities by ensuring access to high-quality, person-centered care that promotes independence, dignity, and community integration. Key Responsibilities Conduct assessments to identify client strengths, needs, and goals Develop, implement, and maintain Person-Directed Plans (PDPs) and Individual Plans of Care (IPCs) in collaboration with the interdisciplinary team Coordinate and monitor delivery of HCS services, including residential, day habilitation, therapy, and community support services Serve as the primary liaison between clients, families, service coordinators, and external providers Complete regular home visits, safety checks, and service reviews to ensure quality and compliance Maintain accurate documentation in accordance with Texas HHS and Medicaid guidelines Track client progress and update care plans based on changing needs Assist clients in accessing community resources that promote independent living Respond to incidents, concerns, or crises and implement appropriate interventions Participate in team meetings, audits, and regulatory reviews Qualifications Bachelors degree in Social Work, Human Services, Psychology, or a related field (preferred but not always required) Experience working with individuals with intellectual or developmental disabilities preferred Knowledge of Texas HCS/TxHmL programs and HHSC regulations is a plus Strong communication, organizational, and problem-solving skills Ability to manage multiple cases and deadlines Proficient in documentation and basic computer systems Valid drivers license and reliable transportation for local travel Core Competencies Client advocacy and person-centered planning Compassion and cultural sensitivity Attention to detail and regulatory compliance Collaboration with interdisciplinary teams Crisis management and decision-making Work Environment Combination of office, community, and client home settings Local travel required within the Waco area Hands-on involvement in residential and community-based care environments Employment Details
