Performs assigned Case Management, skills training, and other related work as required and assigned.EXAMPLES OF WORK PERFORMED
: Provides assessment and treatment with consumers identified for services in the Older Adult Behavioral Health Unit. Possess knowledge and understanding of treatment modalities which will benefit the individual consumer. Possess skills to document clinical services provided in a timely manner no later than 48 hours following interaction with consumer. Participate as a team member on the collaborative team designed for the diagnosis and treatment of consumers in Older Adult population. May supervise other therapists, volunteers and students. Act as liaison between all appropriate agencies, physicians, and community resources for admission, treatment and discharge plans. May participate in budget planning, unit administration, and program development. Increase knowledge of Gerontology, case management, therapy concepts and services to Older Adult consumers, by attendance at work related conferences and seminars whenever possible. HOTBHN provides services in accordance with Trauma Informed Care guidelines. Perform other related work as required and assigned. (above is not all-inclusive list).EDUCATION AND EXPERIENCE
Bachelor's or Master's degree from an accredited college or university in Social Work, Psychology or related field required. Applicants must be able to obtain credentialing as a Qualified Mental Health Professional through training and competency evaluations within 30 days of employment to be able to maintain employment in this position. Qualified Mental Health Professional training is provided through the agency. Two years case management preferred with knowledge of older adult population.KNOWLEDGE, SKILLS, ABILITIES
Employees should have considerable knowledge of the practices, methods, techniques, and resources related to providing case management for older adult individuals living with severe and persistent mental illness. Must have a valid Driver's license and current continued proof of automobile liability insurance. Must have and maintain a level of general health that permits the performance of work duties. Must be able to perform skills necessary for CPR, First Aid, and PMAB certification including but not limited to lifting or supporting at least 30 pounds. Have ability to kneel or stand from a kneeling position. As an employee of the Center, it is my duty to always incorporate the principles of Trauma Informed Care , which include cultural sensitivity throughout the Center, emphasizing a safe and respectful environment for all consumers and staff. HOTBHN shall have a workforce competent in the recognition of and provision of Suicide Safe Care for all consumers in line with national best practices to promote life. Must have excellent verbal/written communication skills. Must have excellent time management and organization/filing skills. Ability to transport clients locally 30% of the time.
