The Homeless Prevention Case Manager provides case management to families who are at imminent risk of homelessness and are living in housing throughout the community. The Case Manager works directly with families to identify barriers to housing stability, develop goals, connect participants with resources, and help families maintain safe and stable housing.

Key Responsibilities:

Conduct assessments to determine program eligibility and complete program intakes. Develop, review, and update Stability Case Plans with participants. Help families identify, prioritize, and work toward short- and long-term goals. Provide case management and conduct home visits. Complete habitability and leadbased paint inspections on rental units Connect participants with community resources and supportive services based on their individual needs. Work with local landlords to advocate for participants and support successful housing retention. Assist households with resolving landlord, neighbor, and other housing-related concerns. Monitor rental payments and other program-related requirements. Maintain effective relationships with landlords, community partners, and service providers. Collaborate with other case managers and agency staff to support participant goals. Maintain accurate participant records, case notes, and documentation in agency databases and case files. Complete required paperwork, reports, grant documentation, and data collection accurately and on time. Monitor participant progress related to income, education, housing stability, and other identified goals. Assist the Housing Coordinator with compiling accurate program data and reports. Participate in homeless street outreach as needed. Attend and actively participate in staff, department, and agency meetings. Perform other duties as assigned.

Strong interpersonal and communication skills. The ability to build positive relationships with participants, landlords, and community partners. Strong problem-solving and decision-making skills. Good organizational and time-management skills. The ability to work independently while also being an effective team member. The ability to maintain accurate and confidential records. A commitment to treating participants with dignity, respect, and compassion. Bilingual English/Spanish skills preffered.

Minimum Requirements:

A high school diploma or equivalent, with two years of relevant education and/or experience. Must be 21 years of age or older. Must have a valid driver's license and clean driving record. Must successfully pass a criminal background check and pre-employment drug screening. Ability to travel throughout the community for home visits, meetings, and other program-related activities using personal and compay vehicle.

Benefits:

Full-time position. Hourly wage based on experience and qualifications. Benefits available in accordance with the Open Doors Personnel Manual for full-time employees.
