This position focuses on coordinating patient cases from referral through discharge, managing scheduling, communicating with referral sources and healthcare providers, and working closely with physical therapists and other case managers to support positive patient outcomes. About MINT Physical Therapy MINT Physical Therapy provides clinic, mobile, workplace, and home-based care throughout Northern Utah. The organization specializes in workplace injury prevention, workers' compensation, motor vehicle accident rehabilitation, and a variety of specialty services. Key Responsibilities Manage patient cases from initial referral through discharge. Coordinate patient care and schedule appointments. Communicate with referral sources, healthcare providers, patients, and members of the care team. Coordinate with physical therapists and other case managers to support optimal patient outcomes. Help maintain organized and timely follow-through on patient cases and referrals. Work effectively across clinic, mobile, workplace, and home-based care settings as needed. Qualifications Strong interpersonal and communication skills. Strong organizational skills and reliable follow-through. Ability to work independently while collaborating effectively with a multidisciplinary team. Adaptable, self-motivated, and comfortable working in multiple care settings. Commitment to ethical, patient-centered care. Spanish-speaking ability is preferred. Compensation & Benefits Full-time position. $18.00-$22.00 per hour. Paid time off.
