Job Description
At First Step House, "We help people build lives of meaning, purpose, and recovery," and we are dedicated to this mission. We are hiring for a full time CHARTER Case Manager in Salt Lake City, Utah!! The CHARTER Case Manager provides comprehensive case management and supportive services as part of a multidisciplinary team. The Case Manager works with individuals experiencing serious mental illness and co‑occurring conditions to support housing stability, recovery, and community integration. This role involves collaborating with residents to identify needs, develop and support service plans, and coordinate daily activities that promote engagement and overall stability. Services are delivered both on‑site and in residents' homes and include individual and group‑based interventions. Case Managers share responsibility for appointment coordination, transportation, and community engagement activities, and assist the Program Manager with administrative tasks as needed. The CHARTER Case Manager is expected to maintain strong professional boundaries, adhere to ethical and legal standards, and work in alignment with organizational priorities established by Senior Leadership, the Executive Director, and the Board of Directors. Case management experience required Experience with mental health and substance use treatment is preferred Duties include but are not limited to: Collaborates with residents to identify needs and supports progress toward individualized service plans Utilizes assertive outreach and supportive engagement to encourage participation with support team Meets regularly with residents to review goals, progress, and evolving needs; including assisting with completion of housing recertification documents to ensure compliance with voucher terms [MI2] Develops, facilitates, and documents life‑skills groups in accordance with program standards Coordinates with community partners to connect residents with employment, education, healthcare, housing, and other supportive resources [MI3] Coordinates and provides transportation to support resident participation in appointments, special events, and community‑based activities that promote engagement and community integration Maintains compliance with HIPAA and applicable 42-CFR regulations Utilizes required software systems and Microsoft Office applications Maintains organizational productivity standards and completes documentation within required timeframes Attends required trainings, staff meetings, and supervision sessions Assists the Program Manager with administrative and program‑support tasks as needed For more information about our non-profit organization, you can visit our website at www.firststephouse.org For more details on testimonials, simply follow the link below and scroll to the bottom.https:
