Job Description
Job Summary This position will work closely with all United Community programs, the Fairfax County Coordinated and Planning (CSP) and Fairfax County Department of Family Services (DFS).
Skills and Abilities Requirement:
Customer Service, excellent communication skills, proficient in MS Word, and Excel, Problem Solving, solution focused, flexible schedule, must have basic data entry skills and management, knowledge in any type of client management Information database systems, positive energy, collaborative, team player, active listening skills, motivational interviewing skills, community organization, adaptable learner, proficiency in additional language is preferred but not required. Reporting and Supervision The Family Achievement Case Manager will report directly to the Family Achievement Program Director who oversees the program. Responsibilities Family Achievement Case Manager will meet the following key responsibilities: Provide holistic, trauma informed, and person-centered therapeutic wrap around case management services to families with the goal of strengthening the family's self-sufficiency and making them workforce ready. Perform career assessments, employment evaluation/review, psychosocial assessments, and evaluations using the Arizona Self-Sufficiency Assessment Scale. Assess, evaluate client skills, create and develop Individualized Goal Plans (IGP) with each family, that include completing career pathways to self-sufficiency and out of poverty guidelines. Conduct client service plan reviews every 3 months and adapt plans to ensure success in reaching their employment objectives. Maintain case files documentation according to the prescribed guidelines. Coordinates and collaborates with team members in assessing client needs, developing and implementing individualized service plans to ensure continuity of care, services and access to community resources. Manage a caseload of up to 30 cases. Maintain well organized client binders with supporting documentation. Provide intensive wrap-around case management services that link participants to key community support services that will help families reach self-sufficiency. Conducts quarterly client events/workshops with the team to promote client self-esteem and improve client engagement and productivity. Trains and supervises student interns in case management and provides support aligned with their learning plans. Help develop, manage and lead a community based "wrap-around" team to best support the family in working towards self-sufficiency Assist the Program Director with completing assessments, evaluations and analysis of data collected to improve outcomes and service delivery Assist participants with applying for and navigating benefit services such as Social Security and Disability Benefits, SNAP and TANF Assist participants with accessing direct emergency assistance that includes food, housing, utilities, medical and dental as well as transportation Assist with data collection, analysis, and program improvement strategies.Education:
Must possess a BSW and MSW in social work. Minimum five years' experience in case management services. Work experience in workforce development is preferred but is not required.Pay:
