Job Description

THIS POSITION IS OPEN TO CURRENT HANOVER COUNTY COMMUNITY SERVICE BOARD EMPLOYEES ONLY
General Description :
This is a management level, professional position. The incumbent performs case management services for adults receiving behavioral health services (to include mental health and/or substance use disorder). These services may be provided in a variety of settings to include an outpatient clinic, in the client's home or in the community. Incumbent also provides administrative supervision for a team of service providers and administrative support staff.
Organization :
The Case Manager Supervisor reports to a Program Coordinator and supervises relevant positions including: Case Managers, Case Aide, Housing Specialists and Peer Recovery Specialists.
Essential Functions :
Serves as a liaison to Crisis and Urgent Care for case management referrals through the Same Day Access process and to support crisis needs of active case management clients, as needed. Provides records management and compliance to include maintenance of accurate records in accordance with relevant procedures and conducting quality improvement activities as it relates case management services Provides technical and training support to staff as needed Supervises employees, performing related human resources functions including routine staff meetings and individual supervision meetings as well as completing necessary paperwork Provides back-up support and coverage within the team which may include direct interaction with adults with serious mental illness and/or substance use disorder and their families Conducts quality improvement activities as it relates to case management services May provide Restoration of Competency services as needed and appropriate Serves as a liaison with other local, regional and state organizations Assesses client and program needs in order to determine service gaps Develops written policies/procedures and operating guidelines as related to case management services. Conducts assessments to determine appropriate level of care and develops individualized service plans Provides behavioral health support to individuals and families Prepares statistical and casework reports as required Works in cooperation with community partners Performs related work as assigned

Environment Office Field Physical Effort Minimal See Special Conditions FLSA Status Exempt Knowledge, Skills, and Abilities:
Requires a thorough working knowledge of program development and management, person-centered planning, and client financial benefits. Requires extensive knowledge of Medicaid and Department of Behavioral Health and Developmental Health Services policies and regulatory guidelines. Must be familiar with methods and theories of evaluation for mental illness, developmental disabilities and substance use disorders, provision of high-quality case management services and implementation of service plans. Ability to maintain confidentiality is required. Excellent oral and written communication skills are required. Ability to respond effectively to crises, make independent decisions, work cooperatively and remain flexible are required.
Education, Experience, and Training:
Bachelor's degree in related field required (Master's degree preferred) with at least four (4) years of progressive related experience, including one (1) year of supervisory experience - OR - Any equivalent combination of education, experience and/or training sufficient to demonstrate the knowledge, skills and abilities is acceptable. Must meet the requirements for or be registered as a QMHP.

Criminal Records Check, including fingerprinting Must obtain valid Driver's License within 30 days of employment to be eligible to operate a county vehicle Twelve-month probationary period CPS (Child Protective Services) Check TB test within 30 days of employment Hepatitis B immunization, recommended Work beyond normal schedule may be required Must be able to successfully pass CPR/First Aide training as well as behavior management as required by regulation, including but not limited to Title 12 of the Virginia Administrative Code. This training process includes a hands-on, in-person demonstration of first aid and CPR competency, for successful completion. This demonstration requires bending, lifting, and completing activities on the floor.
