Job Description

STARS, a group home for teenagers, is currently searching for an enthusiastic and effective case manager.
Job description:
Assist the Program Director with the following, under the Program Director's supervision: Guidance and supervision of the Residents Oversee physical care Development of acceptable habits and attitudes Management of Resident behavior; helping to meet the goals and objectives of any service plan Communication with placing agencies and support agencies Assessments, admissions, service plans, discharges of Residents Crisis intervention Monitoring of all Resident records and program logs Monitor medication administration and documentation Supervision of all medical, educational and therapeutic services Provide 1:1 counseling to Residents not receiving therapy/counseling in the community Help Residents and Parents or Legal Guardian understand the effects on the Resident of separation from the Family and the effect of group living Assist Resident and the Family to maintain their relationships and prepare for the Resident's future care Utilize appropriate community resources to provide services and maintain contacts with such resources Help Resident strengthen their capacity to function productively in interpersonal relationships Confer with Child Care Staff / Residential Counselors to help them understand Resident's needs in order to promote adjustment to group living Work with Resident and the Family or any Placing Agency that may be involved in planning for the Resident's future and in preparing the Resident for the return home or to another Family, for independent living, or for other Residential care All other duties assigned that the Staff is qualified to perform
Qualifications/Education and Experience:
Have a master's degree in social work, psychology or counseling; Have a baccalaureate degree in social work or psychology with documented field work experience and must be supervised by the program director or other Staff employed by the provider with the same qualifications as required by 22 VAC-40-151-290-D; or Have a baccalaureate degree and three years of professional experience working with children.

$46,789.00 - $49,982.00 per year
