The case manager is esponsible for assisting those with pre and post release services and working with the pre and post coordinator to receive participants that are coming out of the city jails, regional jails and state institutions. Duties include meeting with clients and their families, connecting clients with professional services and maintaining communication with the clients to assess their wellbeing and make changes to their treatment plan as needed. Duties and Responsibilities Case Managers perform several tasks to achieve the best outcomes for their clients. This means that you work with clients, jail staff, mental health workers and others. Responsibilities may vary within the criminal justice system, but assessment starts prior to release from the institution or jail. The following occurs prior to and after release from the facilities or direct referral from a court or a probation officer. Assess clients with the OST for risk factors to determine goals, and referrals. Case manager and client co-develop goals and objectives on their Individual Service Plan and Individual Employment Plan factoring in their needs, physical or mental wellness, job skills, preferences and abilities using these to develop tailored treatment plans and reentry program. Evaluate clients' progress daily, weekly, monthly or wherever needed to improve successful outcomes. Following up with satisfication surveys by the clients evaluating their experience of level of services provided by the case manager at their appointment. Knowledge of databse entries documenting clients intake, referrals and follow-up case note. Community Outreach events such as job and resource fairs participation. Ability to facilitate Reentry Curriculum from 8-13 weeks job readiness and discharge planning. Hourly Salary range : $20.00 to $22.00 depending on experience and background. On site positon.
