We are offering a great opportunity for a full-time Case Manager-Justice Services for our Community and Crisis Department. Responsibilities This position serves justice-involved transitional-age youth and adults by providing intensive community-based case management and transition coordination services for those preparing for release from incarceration and reintegrating into the community. The target population for this service is individuals in incarcerated settings and those assigned to specialty docket/diversion programming. The position is responsible for reducing barriers to care, minimizing service fragmentation, and connecting individuals to resources that support stability, recovery, wellness, and long-term independence. The Case Manager plays a critical role in care coordination, advocacy, discharge planning, transition support, and monitoring service engagement throughout the re-entry process. This position requires a highly collaborative, trauma-informed, person-centered, recovery-oriented, and strengths-based approach, with strong knowledge of justice systems, behavioral health systems, and community resources. Required Education and Experience A bachelors degree in human services or a related field. One (1) year of position-related experience providing community-based mental health and case management services to the SMI and SUD populations. Minimally registered as a QMHP-T with the Virginia Board of Counseling or have any equivalent combination of education, training, and experience that provides the required knowledge, abilities, and credentials for the role. All GRCSB employees that provide case management services must complete the case management curriculum developed by DBHDS within 30 days of employment. Valid Virginia Drivers License with a point balance of -6 or higher. Required Knowledge, Skills and Abilities Knowledge of justice-involved populations, behavioral health and co-occurring disorders, case management principles, psychosocial rehabilitation, service planning, documentation requirements, and available community resources and service delivery systems. Knowledge of laws, regulations, and standards related to case management and service delivery. Knowledge of community-based resources relevant to housing, healthcare, behavioral health, benefits, vocational services, and re-entry support. Excellent written, oral, and interpersonal communication skills. Ability to effectively assess needs, coordinate care, problem solve, and support access to crisis and emergency services as needed. Ability to exercise sound judgment, demonstrate initiative, and work independently while maintaining effective communication with supervisors and team members. May be required to travel within the Greater Reach CSB catchment area. Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection.
