Options Counselor Local Office on Aging Roanoke, VA Job Details Full-time $17 - $19 an hour 1 day ago Benefits 403(b) matching Health savings account Disability insurance Health insurance Dental insurance Paid time off Employee assistance program Vision insurance 403(b) Life insurance Qualifications Teamwork Social work Driver's License Bachelor's degree Case management Human Services Home & community care experience Full Job Description Position Summary The Elder Care Options Counselor provides person-centered counseling, care coordination, and resource navigation to older adults, individuals with disabilities, caregivers, and families. This role helps individuals make informed decisions about long-term services and supports that promote independence, dignity, safety, and community living. The ideal candidate is compassionate, organized, resourceful, and committed to helping individuals access services that improve quality of life. Person-Centered Counseling & Support Conduct assessments to identify client needs, strengths, preferences, and risks Help individuals develop person-centered service plans based on their goals and choices Provide information about available long-term services, supports, and community resources Assist clients and caregivers in making informed decisions regarding care and support options Conduct reassessments as required Care Coordination & Resource Navigation Coordinate access to programs and services including: Nutrition assistance Transportation In-home services Benefits programs Community resources Make referrals and follow up to ensure service connection Advocate for access to appropriate services and supports Documentation & Compliance Maintain accurate, timely, and audit-ready client records Document assessments, referrals, service plans, and follow-up activities Utilize required state systems, including Peer Place Ensure compliance with DARS standards, confidentiality requirements, and agency policies Community Outreach Participate in community outreach, presentations, and awareness activities Build collaborative relationships with service providers and community partners Qualifications, Education & Experience Bachelor's degree in Social Work, Human Services, Gerontology, Counseling, or related field is preferred Two (2) to five (5) years of experience in case management, counseling, social services, or related human services field preferred Experience working with older adults or individuals with disabilities strongly preferred Equivalent combinations of education and experience may be considered Knowledge, Skills & Abilities Knowledge of aging services, community resources, and long-term services and supports (LTSS) Strong communication, interpersonal, and problem-solving skills Ability to manage multiple cases and deadlines effectively Proficiency with computer systems and data entry Ability to maintain confidentiality and professional boundaries Commitment to person-centered service delivery Additional Requirements Valid Virginia driver's license required Reliable transportation for in-home visits and community outreach Ability to travel throughout the 5th Planning District Ability to conduct home visits and work in various community environments Ability to sit, stand, and use computer systems for extended periods Why Join Our Team?
