Job Description

Background:
This role supports the Recovery Navigator Program , created in response to the Blake decision and launched through SB 5476 , with continued support from 2E2SSB 5536 . You'll work closely with law enforcement and community partners to divert individuals from jail and guide them toward recovery services. This position offers a unique opportunity to support people impacted by substance use and help them find a new path forward.
Summary:

The Outreach and Referral Coordinator serves as the primary contact for all referrals to the Recovery Navigator Program and Destination Hope and Recovery. This role ensures smooth communication between referral partners and provides outreach, screening, and engagement for individuals referred to the program. Duties include conducting street outreach when needed, building trust with participants, and connecting them to appropriate community resources. Strong collaboration with service providers and community partners is essential to support participant success.
Job Duties:
  • Respond to community referrals and interact with individuals who might need case management or ongoing referrals to external services.
  • Provide short-term assistance while addressing the immediate needs of the individual.
  • Facilitate warm handoffs to the support services identified.
  • Follow up with program participants in the community when there is an indication of disengagement.
  • Prioritize responding to law enforcement calls, while also being able to respond to any community-based and emergency response referral.
  • Coordination with case management staff to meet the individual needs of new and existing program participants.
  • Utilize conflict resolution and de-escalation techniques when necessary to defuse individuals in crisis.
Requirements and Preferred Qualifications:
Preferred Education:
Minimum High School Diploma or GED equivalent. Valid Drivers License and Insurance
  • This is an office/field based position , which includes regular travel within Lewis County, as well as periodic travel to other cities and counties in Western Washington.
