00 per Hour Company Overview Consistent Care Support Services is dedicated to serving and advocating for individuals with complex social, behavioral health, housing, and employment needs. Through compassionate, community-based case management, we assist individuals in overcoming barriers to stability while supporting long-term recovery, housing retention, employment success, and overall well-being. Position Summary We are seeking a compassionate, motivated, and solutions-focused Case Manager to support individuals residing in recovery residences and supportive housing environments. This position works closely with clients experiencing homelessness, substance use disorders, mental health challenges, and other barriers to self-sufficiency. The Case Manager will provide intensive case management, housing stabilization services, employment support, resource navigation, advocacy, crisis intervention, and recovery-focused support designed to help clients achieve and maintain stability, independence, and improved quality of life. Essential Duties and Responsibilities Conduct comprehensive assessments of client needs, strengths, barriers, and goals. Develop individualized service plans focused on housing stability, employment, recovery, and overall wellness. Assist clients in obtaining and maintaining safe, stable housing. Support clients in securing employment, vocational training, educational opportunities, and income resources. Provide ongoing case management services through home visits, community outreach, and office-based appointments. Coordinate services with healthcare providers, behavioral health professionals, landlords, employers, treatment providers, and community partners. Provide crisis intervention, de-escalation, and problem-solving support when needed. Utilize motivational interviewing and person-centered approaches to promote engagement and positive outcomes. Assist clients with applications for benefits, identification documents, transportation resources, and other essential services. Maintain accurate, timely, and audit-ready documentation in accordance with company, contractual, and regulatory requirements. Advocate for clients while promoting personal responsibility, self-sufficiency, and recovery-oriented goals. Participate in team meetings, supervision, training, and quality assurance activities. Qualifications Bachelor's degree in Social Work, Human Services, Psychology, Behavioral Health, or a related field preferred. Relevant experience may be considered in lieu of education. Experience working with individuals experiencing homelessness, substance use disorders, mental health challenges, or other vulnerable populations preferred. Knowledge of housing resources, employment services, behavioral health systems, and community-based support programs. Strong crisis intervention, conflict resolution, and problem-solving skills. Ability to work independently while maintaining effective communication with supervisors and team members. Excellent documentation, organizational, and time-management skills. Ability to establish professional boundaries while building positive, supportive relationships with clients. Valid driver's license, reliable transportation, and proof of automobile insurance required. Knowledge, Skills, and Abilities Strong client advocacy and relationship-building skills. Understanding of recovery-oriented and trauma-informed care principles. Ability to navigate complex systems and connect clients to appropriate resources. Strong verbal and written communication skills. Ability to manage multiple priorities in a fast-paced environment.
