They connect residents to community resources, assist with vocational and educational goals, and oversee individualized recovery plans to maintain a stable household environment.
Job Summary:
seeking a compassionate, organized, and boundary-driven Case Manager to support our residents as they transition from primary treatment into long-term recovery. In this role, you will act as a personal advocate and guide, helping clients navigate the complexities of outpatient care, employment, legal requirements, and daily independent living. You will collaborate closely with house operators and clinical teams to foster a safe, accountable, and supportive environment. Key ResponsibilitiesClient Assessment & Service Planning Conduct intake interviews and needs assessments for incoming residents. Collaborate with clients to build and review individualized recovery/service plans, tracking short- and long-term milestones. Monitor progress through regular, one-on-one check-ins. Resource Coordination & Advocacy Connect residents with essential community services, including outpatient therapy, psychiatric care, and 12-step or SMART recovery groups. Assist clients in securing employment, resume building, and vocational training. Act as a liaison between residents, legal systems (e.g., courts, probation officers), and family members. Accountability & Program Compliance Educate clients on house rules, curfews, and community expectations. Maintain accurate, confidential records of resident progress, incident reports, and case notes. Coordinate with house management staff regarding drug screenings, breathalyzer testing, and room checks. Crisis Intervention Provide rapid crisis de-escalation and support during periods of high stress, conflict between residents, or relapse events. Facilitate safe and documented discharge processes if a resident leaves the program.
Qualifications & Requirements Education:
Bachelor's degree in Social Work, Psychology, Counseling, or a related field (Preferred).
Experience:
1-3 years of experience in substance use disorder treatment, behavioral health, or sober living.
Lived Experience:
Individuals with lived experience in addiction recovery are highly encouraged to apply, typically requiring a minimum of 5 years of continuous sobriety.
Certifications:
Certified Peer Recovery Specialist (CPRS), Certified Chemical Dependency Counselor (CCDC), or equivalent credentials are a plus.
Skills:
Strong conflict resolution, excellent boundary-setting, and clear understanding of addiction, relapse triggers, and 12-step/alternative recovery philosophies.
Compliance:
Ability to pass a background check and drug screening.
