SOAR is seeking a Community-Based Case Manager who is passionate about providing high-quality, community-based direct case management support services while making a meaningful impact in people's lives. This is a full-time position working 40 hours per week. Typical hours are Monday through Friday, with weekend and evening on-call hours scheduled as assigned based on staff rotation. This position does work in person at our Madison location and may travel in the Dane County area to meet with clients.
The Community-Based Case Manager will:
Provide direct case management support to persons served while building meaningful relationships based on trust, mutual respect, and uniqueness of lived experience. Support individuals in achieving self-identified goals, increasing their independence, cultivating resilience, developing new skills, learning new abilities, and enhancing their overall quality of life. Complete all corresponding Comprehensive Community Services (CCS), Targeted Case Management (TCM) assessments & other treatment plans as required per individual. Coordinate care for persons served with various community supports. Develop and maintain positive relationships with community providers to best support persons served. Assist persons served with weekly medication observation. Complete and manage intakes, assessments, treatment plans & Ch. 35 plans, and all necessary documentation accurately and timely. Maintain daily documentation of services provided Maintain billable hours per year in accordance with agency requirements. Attend and actively participate in regularly scheduled meetings. The Community-Based Case Manager will need: Bachelor's Degree in Psychology, Human Services, Social Work or related field is required. Demonstrated ability to provide client care in a social service setting. Proficient with Google Workspace and electronic documentation systems. Ability to effectively communicate in verbal and written form. Ability to work in a multidisciplinary team with a positive attitude. Strong skills in conflict resolution, boundary setting, and interpersonal communication. Must be able to provide documentation with degree confer date & transcript at time of hire. Must possess a valid driver's license, insurance to meet state requirements, and access to a vehicle to use for work purposes. Must be able to successfully complete a caregiver background check at hire and annually thereafter in accordance with the state and local DHS requirements. Must respect and comply with organizational health and safety practices while honoring the personal health decisions of clients and staff. In exchange, the Community-Based Case Manager will receive compensation of $21.00 per hour.
