Do you have Case Manager Experience? This position is 25 hours per week at our new Marshall, WI location. Occasional evenings and weekends. Have you helped people walk in transition make lasting change in their lives? You must be a practicing Christian to work in our faith based ministry. Must believe that transformation is possible, even if others don't. a strong Christian Faith as this is a ministry position Our next Care Partner/Case Manager will walk alongside women living in our residential maternity home and help them build the resources they need to thrive. Some duties of the Case Manager for The Elizabeth House maternity and parenting home include: Responsible for weekly sessions with each resident to support the move toward self-sufficiency and independence. In partnership with Program Director, oversee the participation of residents in program activities. Monitor compliance that each resident keeps her prenatal care and child's pediatric appointments Assist each resident in the development and coordination of education, employment, legal, financial, and parenting goals. Be an advocate and liaison on behalf of the residents and the program with social service agencies and families. Help them learn how to effectively advocate for themselves and their children through example setting, mutual strategization, and encouragement. Be responsible for maintaining residents' files, progress reports, and other paper work. Complete assessments as required. Participate in applicant process and interviews to determine eligibility for The Elizabeth House program in conjunction with the Program Director when needed. Offer alternative resources and referrals for those not admitted. Responsible for transition planning and follow-up for each resident. Work with resident to ensure they are prepared to live independently and have adequate support systems. Provide post-graduation support and check-ins for 3 years. Maintain confidentiality for every resident and her child. Refer residents for appropriate counseling and to assist with working through issues that affect her ability to have healthy personal relationships, and move forward in program and life goals. Have working knowledge of Dane County's resources as it relates to housing, employment, education, child-care, and economic stability Weekly meeting with Program Director. Provide appropriate Christian spiritual guidance related to daily living and parenting. Lead and encourage participation in components of spiritual growth, including daily devotions, spiritual discussions and other components of spiritual growth plan. Work with other staff to establish a plan with each resident for regular participation in education program, including pregnancy, childbirth, infant safety, and parenting, as well as Healthy Relationships. Document compliance with plan. Facilitate education on parenting, pregnancy classes and healthy relationships, life skills and finance with residents as needed through approved educational platforms Participate with TEH staff and seasoned volunteers in occasional evening, weekend, and/or holiday rotation coverage as needed to maintain residence coverage Oversee and prepare chore and meal rotation for residents. Help with meal planning, nutrition education. Support those needed help with cooking duties. Monitor house supplies and pantry needs weekly. Prepare a list of needs for purchase for the Program Director. Assist and orient new residents with becoming acquainted with the daily schedule, operations, and expectations of the program. (dishwasher, laundry, computers, etc) Conduct weekly room checks and conduct room searches if warranted. Work with the program director to plan house activities, including birthday parties, holiday celebrations, trips outside the home, resident graduation parties, etc. Transport residents to programmatic activities including work, medical or social service appointments. Be available to the residents to support and encourage them during your shift, looking for teaching moments at every opportunity.
