Job Description
D ue to the expansion we are in need for another Service Coordinator to provide case management services for the Children's Long Term Support Waiver Program. This is a Full Time position managing a case load of up to 40 children. The goal of this program is to provide services that will keep the child living in their natural environment. The position will be based in Racine County. A Bachelor's degree in a Human Services field, and at least one year experience working with children and/or youth with disabilities is required. Knowledge of the CLTS Functional Screen and/or CLTSWaiver program is preferred. A valid driver's license is required. Position available immediately.
