FAMILY PROMISE OF WASHINGTON COUNTY JOB DESCRIPTION - CASE MANAGER FULL TIME POSITION JOB TITLE
Case Manager

JOB SUMMARY
This position provides a full range of case management for persons who are homeless or at risk of being homeless.
ESSENTIAL FUNCTIONS
  • Interview and evaluate clients to complete the intake process.
  • Utilize advanced knowledge to:
  • Screen and assess eligibility for requested services for homeless clients.
  • Complete comprehensive assessments.
  • Develop and implement Family Promise orientation program for clients.
  • Analyze guest situations, capabilities, and problems to determine services required to meet program needs of clients.
  • Formulate, implement, evaluate, and revise goal oriented and strength-based action plans.
  • Establish and implement program expectations.
  • Provide timely crisis intervention to ensure client success.
  • Develop a crisis/safety exit plan for the clients.
  • Exercise judgment to intervene as an advocate for clients in emergency and crisis situations.
  • Provide training to new staff and interns.
  • Formulate daily contacts into database and meet all deadlines of documentation including assessments, action plans, crisis plans, monthly reports and evaluations.
  • Effectively engage clients to promote change to improve functioning and overall home environment in the program and upon exit of the program.
  • Provide information and referral services to help clients access other community resources.
  • Work in collaboration with other providers, school personnel, mental health professionals, and Department of Human Services. Attend meetings as necessary.
  • Provide clear communication of client schedules and profiles with volunteers.
  • Attend promptly weekly staff/monthly volunteer coordinator meetings and other meetings as needed.
  • Complete proper documentation and file information as outlined by programmatic procedures, including all required reports for federal and local funding.
  • Responsible for cleanliness and general maintenance of Shelter, Day Center/Resource Center, Apartments, Building, Yard, and Parking Lot.
  • Responsible for routine weekly checks in the Shelter Areas, Day Center and Apartments.
  • Responsible for clients adhering to program rules.
  • Maintain regular contact with volunteers and other individuals at each support congregation, as necessary.
  • Participate in the organization's fundraising events as a volunteer and represent the organization with positivity.
  • Safely transport clients as needed.
  • Other duties as assigned.
Education and Training Preferred:
Bachelor's degree in Social Work, Psychology, Counseling or other related field. Position must keep current on knowledge of current local, state and federal laws, and all rules and regulations.
Experience Preferred:
1 year of experience in this field or related field.
Knowledge, Skills and Abilities:
  • Ability to read, speak and write clearly in English.
  • Ability to exercise good judgment and discretion in establishing work schedule.
  • Must be able to work cooperatively and collaboratively with Workforce Development Center, Washington County Human Services and other community agencies.
  • Must establish and maintain productive and professional relationships with clients, colleagues, and other community agencies.
  • Must maintain client confidentiality.
  • Ability to prioritize and make appropriate decisions.
  • Ability to work independently and effectively manage time.
  • Must possess strong oral and written communication skills.
  • Ability to provide culturally, ethnically and gender sensitive services.
  • Ability to follow directions and implement recommendations.
  • Ability to identify problems and solutions.
  • Ability to self-motivate and remain on task.
  • Knowledge and compliance with policies and procedures as detailed in the personnel policy and procedure manual.
  • Ability to operate standard office equipment.
  • Ability to operate a motor vehicle safely and obey all traffic laws. Must have reliable transportation on a daily basis, a valid driver's license and required automobile insurance. Other Information
  • Occasional evening and weekend hours required based on the needs of the program.
  • Maintain a caseload as assigned.
  • Ability to sit, stand and drive throughout the day.
  • Ability to lift and move objects weighing up to 50 pounds.
  • Must possess visual acuity.
Job Type:
Full-time Pay:
$42,000.00 - $55,000.00 per year
