Job Description
The goal for the case manager position is to accomplish residents' care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; and facilitating interdisciplinary approaches. Bachelors degree is required. General case management duties, treatment planning, report writing, assessment and referral responsibilities. Meeting and court attendance is also required. This position deals with youth in residential treatment facility between ages of 12-17.
