Job Description

Job Summary We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by coordinating services, advocating for their needs, and facilitating access to resources. The Case Manager will work collaboratively with clients to develop personalized care plans that promote well-being and stability. Duties Conduct comprehensive assessments to identify client needs and develop individualized service plans. Provide ongoing support and guidance to clients, ensuring they have access to necessary resources. Collaborate with healthcare providers, social services, and community organizations to coordinate care. Monitor client progress and adjust service plans as needed to achieve desired outcomes. Utilize motivational interviewing techniques to empower clients in making positive changes. Maintain accurate documentation of client interactions, progress notes, and case files in compliance with agency policies. Supervise support staff as needed and provide training on best practices in case management. Experience A background in social work or a related field is preferred. Familiarity with addiction counseling techniques is a plus. Strong communication skills are essential for effective collaboration with clients and external partners. Ability to manage multiple cases simultaneously while maintaining attention to detail. Associate's Degree in a related field is required.
