A Case Worker is employed by a government agency, non-profit organization, or other social services group to take on the cause of individuals and provide them with advocacy, information, and other services.
The Wraparound Facilitator will establish a participant-centered process for facilitating supports, range of services, communication between participant's families and service providers, and assistance to participants in the Children with Serious Emotional Disorders Waiver (CSEDW) and Safe-at-Home.
Job Duties:
Initiate services within 72 hours for referrals received by Aetna Better Health of WV (CMO for CSEDW program). Collaborate with Aetna case manager assigned to the CSEDW participate for coordination of care. Complete weekly home visits at participant's natural family home or foster placement. Facilitate POC meetings every 30-45 days (12 POCs per year). Conduct the CANS assessment as part of wraparound facilitator model ongoing progress monitoring every 90-days. Facilitate re-determination meetings for CSEDW programs annually with Acentra. Check Medicaid eligibility monthly. Facilitate discharge process and provide linkage to services appropriate to level of need for participation. Assist with procurement of all services that are appropriate and necessary for each participant in the CSEDW and SAH programs. Provide education, linkage, and referral to community resources for the youth and family. Comply with all company/state/federal reporting requirements. Act as an advocate for the participating CSEDW and SAH members Facilitate Significant life event meetings within 48 hours of a critical incident report or grievance family may have with therapeutic services. Facilitate writing of the wraparound Plan of Care (POC) during team meetings. Coordinate treatment plan monitoring with therapeutic team members. Facilitate transfer meetings for services listed in FOC (freedom of Choice) for the participant. Access services detailed in the POC, make referrals to qualified service providers and resources, and monitor service providers implementation the treatment plan therapy objectives listed in the PCSP. Disseminate copies of the POC, CANS, transfer papers, discharge papers to team members within 14 days of the meetings date. Ensure documentation is uploaded into the WV CANS network for assigned cases. Provide planning and coordination before, during and after a crisis. On-call duties relating to crises of youth on assigned caseloads.
Qualifications:
Have a bachelor's degree plus 1-years' experience working with individuals with emotional or behavior disorders or foster care system; or an associate's degree in human services field with 2-years post graduate work experience in the field. LSW are encouraged to apply. Excellent verbal and written communication skills. Ability to travel between locations (valid driver's license, registration, and automobile insurance). Knowledge of Microsoft 360 word, excel, outlook calendar, and TEAMS. Ability to multi-task, coordinate, run, and mediate CFT meetings CPR/FA certification Complete of all CSEDW required training pertaining to licensure.
Preferred Trainings upon hire:
Non-LSW's, RNs, and licensed professional counselors must hold a current conflict -free case management certificate from BMS. Hold CANS and/or CAFAS assessment certification.
