  • Full-time Overview Join our dynamic team as a Case Manager, where your expertise will drive client-centered care and ensure seamless coordination across settings.
In this vital role, you will assess patient needs, develop comprehensive treatment plans, and collaborate with multidisciplinary teams to optimize health outcomes. Duties Conduct thorough client assessments to determine medical, psychosocial, and discharge planning needs using EMR (Electronic Medical Record) and EHR (Electronic Health Record) systems. Develop individualized treatment plans that incorporate protocols, behavioral health considerations, and managed care requirements. Oversee utilization review and management processes to ensure appropriate resource use while maintaining compliance with HIPAA regulations and medical terminology standards. Facilitate discharge planning by collaborating with social workers, physicians, nurses, and other healthcare providers to ensure smooth transitions from hospital to home or other care facilities. Support patient service initiatives by providing education on treatment plans, medication management, and post-discharge instructions tailored to pediatric or adult populations. Skills Familiarity with EMR/EHR systems used for documentation, case management, and discharge planning. Excellent communication skills for effective collaboration with multidisciplinary teams including social workers and behavioral health specialists. Ability to navigate managed care policies while advocating for patient needs within compliance standards such as HIPAA. Strong organizational skills for managing multiple cases pressure environments.
