Campbell Ave., Tucson, AZ 85719 USA Position Highlights The Community Outreach Assistant (II) will play a vital role by assisting the Wellness Manager of the Petersen HIV Clinics to help vulnerable patients to link to needed community resources. The Community Outreach Assistant will help identify community-based interventions and resources to assist a diverse patient population facing medical and psychosocial needs around southern Arizona. The Community Outreach Assistant will help to maintain project data entry and resource library. The position responsibilities are routine in nature and will receive guidance especially for new tasks as well as ongoing support daily. This is a part time position that may end 5/31/2027. Duties & Responsibilities Under supervision of the Wellness Manager, help patients access community resources by identifying appropriate services and providing necessary referrals. Ensure that patients receive the support they need by connecting them with relevant external programs and services. Participate in care planning and follow-up activities through in-person, virtual, and phone interactions with patients. Provide administrative support during these sessions by assisting with scheduling, documentation, and coordination efforts to facilitate effective care and resource utilization. Provide targeted data collection and monitoring of behavioral health referrals to improve Petersen HIV Clinics retention and viral load suppression rates for patients reporting behavioral health conditions. Assist the Wellness Manager to outreach to current and new resources in the southern Arizona communities to forge partnerships with behavioral health agencies to promote health equity and care coordination for patients. Organize partner meetings to review partnership needs and service updates, i.e., meet and greet, phone call or virtual meeting. Assist the Associate Director in the pursuit of formal agreements with collaborations i.e., letters of agreement or MOUs. Maintain resource guide for Petersen HIV Clinics staff and patients that identify behavioral health support services and other resources within Petersen HIV Clinics' service area.
