Patient management software High school diploma or GED Clinical information systems Computer skills Clinical confidentiality policies Resume management software Cross-functional communication Full Job Description CareNationDC is seeking a strong, organized, and community-focused Outreach Coordinator to support referral development, community partnerships, recruitment coordination, and outreach reporting for the agency's Mental Health Rehabilitation Services and Substance Use Disorder programs. The Outreach Coordinator will be responsible for building relationships with referral sources, helping increase client referrals, coordinating recruitment leads for Recovery Support Specialists, Community Support Workers, and other program staff, and keeping leadership informed about current outreach trends, referral patterns, community needs, and relevant regulatory expectations. This position requires someone who is professional, dependable, good with people, comfortable representing the agency in the community, and able to follow up consistently with referral partners, applicants, clients, and internal team members. Key Responsibilities The Outreach Coordinator will be responsible for: Developing and maintaining referral relationships with hospitals, clinics, shelters, schools, community organizations, churches, courts, probation/parole offices, housing programs, social service agencies, and other community partners. Promoting CareNationDC's MHRS, SUD, psychiatric, case management, therapy, and support services to appropriate referral sources. Receiving, tracking, and following up on referrals to ensure clients are connected to intake and services quickly. Coordinating with the Intake Manager to ensure new referrals complete required intake paperwork, eligibility screening, assessment scheduling, and admission steps. Maintaining a referral tracking log that includes referral source, date received, client status, service interest, followup needs, and admission outcome. Supporting recruitment coordination for Recovery Support Specialists, Community Support Workers, case managers, and other program positions. Identifying qualified candidates through job boards, community networks, workforce programs, colleges, training programs, and professional contacts. Screening basic candidate qualifications and coordinating interviews with the appropriate hiring manager. Helping organize hiring pipelines for RSS, CSW, and other direct service positions based on program staffing needs. Reporting weekly or monthly on outreach activity, referral trends, recruitment leads, conversion rates, community feedback, and barriers to enrollment. Monitoring community needs and outreach trends related to mental health, substance use, housing instability, youth services, crisis needs, Medicaid access, and behavioral health service demand. Staying informed about DBH, Medicaid, HIPAA, client rights, confidentiality, referral, outreach, and marketing expectations that may affect community engagement. Ensuring all outreach activities are ethical, professional, compliant, and respectful of client choice. Attending community meetings, resource fairs, provider meetings, networking events, and agencyapproved outreach activities. Preparing outreach materials, referral packets, service flyers, recruitment information, and partner communication as needed. Coordinating with leadership, intake, clinical, QA/QI, billing, and administrative teams to ensure referrals are handled properly. Protecting confidential client, applicant, employee, referral source, and agency information at all times. Representing CareNationDC professionally in all community, recruitment, and referral settings. Minimum Qualifications Qualified candidates should have: High school diploma required; associate or bachelor's degree in human services, public health, social work, psychology, marketing, communications, healthcare administration, or related field preferred. At least 12 years of experience in outreach, recruitment, case management, admissions, healthcare, behavioral health, social services, community engagement, or customer relations. Experience working in mental health, SUD, community health, Medicaid, social services, or healthcare preferred. Strong communication, followup, and relationshipbuilding skills. Ability to track referrals, recruitment leads, and outreach activity accurately. Ability to speak professionally with referral partners, applicants, clients, families, and internal staff. Understanding of HIPAA, confidentiality, client dignity, and professional boundaries. Ability to multitask and meet deadlines. Basic computer skills, including email, spreadsheets, EHR systems, applicant tracking systems, or referral logs. Reliable transportation preferred if community outreach visits are required. Preferred Qualifications Preferred candidates will have: Experience with DBH, MHRS, SUD, Medicaid, or community behavioral health programs. Existing relationships with community referral sources in Washington, DC. Experience recruiting or coordinating interviews for RSS, CSW, case management, or behavioral health positions. Experience attending resource fairs, community meetings, and provider networking events. Ability to prepare simple outreach reports and trend summaries for leadership. Familiarity with Credible EHR, Insync, or similar behavioral health systems. Bilingual skills preferred but not required. Can turn community relationships into appropriate referrals. Can support both client referral growth and staff recruitment coordination. Is organized with logs, spreadsheets, reports, and followup tasks. Communicates clearly with leadership and internal departments. Understands confidentiality and does not disclose client or agency information improperly. Can identify trends in referrals, staffing needs, and community demand. Represents CareNationDC with professionalism and integrity. Is comfortable working in a fastpaced behavioral health environment.
