The Wilmington Street Team Ambassador is responsible for providing wraparound services, also referred to as Behavioral Health Rehabilitative Services (BHRS), which are individualized mental health and behavioral health services provided in the home, school, or community to enhance well-being. Since Wilmington Street Team works closely with the community, they should have many skills relating to organization and public engagement.

Case Management:
Provide comprehensive case management services to community members, focusing on wraparound support. Assist community members with applying, registering, or completing forms for wraparound connection. Document all contact with community members in the Apricot system (non-identifying documentation). Provide comprehensive case management services to community members, focusing on wraparound support. Assist community members with applying, registering, or completing forms for wraparound connection. Document all contact with community members in the Apricot system (non-identifying documentation).
Outreach:
Conduct outreach activities to identify individuals and families at high risk of community-based violence. Build trusting relationships with community members to provide support and resources.
Events:
Connect individuals on YAP, CFSE, and NC caseloads to organizational services and events (healing circles, group activities, events, etc.). Post information about these events monthly to the entire WST team.
Safe Passage & Community Walks:
Provide safe passage services to the North and East sides of Wilmington, between the hours of 7:30 am - 9 am and 2:30 pm - 6 pm, in partnership with the Community Education Building. Participate in collective community walks as needed. Schedule community walk participation at least two weeks in advance for all WST collaborative efforts.
Meetings:
Participate in ongoing coordination meetings, trainings, and coaching with Wilmington CSPI Coordinator Dr. Debra Mason and the Community Based Public Safety Collective. Attend Wilmington Public Safety Round Table meetings, prepared to report on current activities by the organization in the assigned target area.
Data Management:
Enter data into the Apricot system daily upon contact with community members for case management. Complete Apricot training prior to managing wraparound and case management services.
Follow-Up:
Follow up with each community members for three weeks via phone or in person to ensure the connection is maintained. Close the community member from the caseload in week 3 of a successful resource connection, notifying YAP or CFSE for continued support and exit plan. Conduct outreach activities to identify individuals and families at high risk of community-based violence. Build trusting relationships with community members to provide support and resources.
Events:

Qualifications:
Bachelor's degree in Social Work, Psychology, Behavioral Health, or a related field is preferred. Proven experience in case management, outreach, or a related role. Strong communication and interpersonal skills. Ability to work independently and as part of a team. Proficiency in data management and reporting. This position have on call responsibilities on weekends.
Pay:
$40,000.00 - $45,000.00 per year
