Role:
As an Outreach Coordinator, you will serve as a bridge between the community and essential behavioral health services. This role is highly community-focused and allows you to meet individuals where they are-whether in shelters, on the street, or in other community settings-and help guide them toward care and resources. You'll work both independently and alongside a multidisciplinary team to assess needs, remove barriers, and connect individuals to housing, treatment, and supportive services. This is a dynamic, purpose-driven role ideal for someone who is passionate about advocacy, relationship-building, and making a lasting impact in the lives of others. What You'll Do Conduct outreach in the community (shelters, clinics, and other settings) to engage individuals experiencing homelessness Assess needs and connect individuals to appropriate behavioral health and support services Collaborate with a multidisciplinary team and community partners to coordinate care Serve as an advocate, helping clients navigate systems and access housing, employment, and treatment resources Build and maintain relationships with referral sources, community agencies, and local stakeholders Develop and track service plans with clients, supporting progress toward stability and self-sufficiency Maintain accurate and timely documentation in client records and the EMR Participate in outreach events and community engagement efforts to expand resources and awareness What We're Looking For High School Diploma required; Associate's degree preferred Experience working with individuals experiencing homelessness, mental health, or substance use challenges preferred Strong interpersonal and communication skills with a passion for helping others Ability to work independently in community-based settings while collaborating with a team Knowledge of community resources and service systems is a plus Valid Florida Driver's License with acceptable driving record Must be at least 21 years of age and meet company insurance requirements Ability to travel locally to conduct outreach and attend community events
