Job Description

Job Summary The Wadena HRA Community Outreach Coordinator is responsible for planning and assisting clients with services to enable independent living, aging in place and meet goals and objectives for the residents of the Wadena Community. Connecting participants with needed resources and coordinating events and activities with existing community partnerships while continuing to develop new partnerships within the community with the objective to increase self-sufficiency and housing stability. Tracking and evaluating program success and preparing program/grant reports. Assist with receptionist/office duties as needed. Duties Develop and maintain a comprehensive network for resources in health and wellness, living skills, nutritional assistance programs, broadband internet, other supportive services, drug and alcohol abuse. Generate interest and use of community resources. Provide clear and useful program information that informs potential candidates and motivates participation. Identify/Evaluate applicant/participant needs, define clear and measurable goals to address those needs and connect with resources. Document progress and re-evaluate services for adequacy. Assist participant in completing necessary applications, enrollment forms and other necessary documents. Assist participants in building informal support networks with family, and friends. Have regular contact with participant including in person, phone, email, and in writing. Monitor delivery of ongoing services to ensure needs are being met. Accurately complete paperwork, and case notes on a timely basis. Greet public, screen telephone calls and direct clients to the appropriate staff. Perform tasks in accordance with standards established by regulations, policy and procedures. Provide clerical functions to Wadena HRA staff. Collect payments, make bank deposits, enter receipts in housing software, balance, and close monthly rental accounts. Create, revise, and update forms, correspondence, and other materials as needed. Maintain resource materials as directed by program managers. Monitor and maintain social media and website accounts. Maintain and value confidentiality. Pay:
$18.77 - $25.14 per hour
