Full Job Description Position Summary:
The Outreach Coordinator is responsible for building and maintaining relationships with referral sources, community partners, care coordinators, hospitals, social workers, and other stakeholders to increase awareness of agency services and support growth. This role serves as a liaison between the agency and the community, helping identify referral opportunities and ensuring excellent communication with prospective members and families.
Key Responsibilities :
Develop and maintain referral source relationships. Conduct outreach to hospitals, care management agencies, DSS, schools, and community organizations. Represent the agency at community events, networking meetings, and resource fairs. Track referral activity and maintain outreach records. Follow up on referrals and assist with intake coordination. Distribute marketing materials and agency information. Collaborate with leadership to identify growth opportunities. Promote agency services in compliance with NC Medicaid and DHHS requirements. Assist with community education and presentations.
Qualifications :
Associate's or Bachelor's degree preferred. Experience in healthcare, behavioral health, IDD services, marketing, or community relations preferred. Strong communication and relationship-building skills. Valid driver's license and reliable transportation. Proficient with Microsoft Office and CRM/database systems.
