Job Description
Community Outreach Representative We are looking for an energetic, outgoing, and self-motivated Community Outreach Representative to build relationships with healthcare providers and community organizations throughout the Hudson Valley. This position is ideal for someone who enjoys meeting new people, networking, and making a positive impact in the community. The primary responsibility is to educate organizations about our Medicaid-covered meal program and encourage referrals for eligible individuals and families. Responsibilities Visit hospitals, medical practices, care management agencies, behavioral health providers, nonprofits, food pantries, senior centers, shelters, and other community organizations. Build and maintain professional relationships with referral partners. Present information about our Medicaid meal program and answer questions. Distribute brochures, flyers, and marketing materials. Attend community events, health fairs, and networking opportunities. Identify new referral sources throughout the Hudson Valley. Maintain accurate records of outreach activities and follow-up visits. Represent the organization professionally at all times. Qualifications Excellent communication and interpersonal skills. Friendly, confident, and comfortable speaking with professionals. Strong organizational and time management skills. Self-motivated and able to work independently. Reliable transportation and a valid driver's license. Willingness to travel throughout the Hudson Valley. Experience in community outreach, healthcare, sales, nonprofit organizations, or customer relations is preferred but not required.
