As the Street Outreach Coordinator, you will play a pivotal role in individuals' lives, making a significant impact through: Conduct outreach to locate individuals and families experiencing homelessness or housing instability in the community Build and maintain relationships with shelters, landlords and community partners Participate in community meetings as requested Assist with monitoring the Day Resource Room one time per week, as necessary Assist the Case Managers as requested by

Street Outreach Coordinator Schedule:

Monday

• Wednesday, Friday 8:30 AM
• 4:30 PM, Thursday 8:30 AM
• 6:30 PM (35 hours per week)

Qualifications:
