About the Role We are seeking a relationship-driven, community-minded Community Outreach & Referral Coordinator to serve as the face of Happy Seniors Homecare throughout the communities we serve. This position is responsible for building referral relationships, increasing community awareness, developing strategic partnerships, and helping connect seniors and their families with quality home care services. This is a primarily field-based position requiring frequent local travel. Approximately 80% of your time will be spent in the community building relationships, attending networking events, and developing referral partnerships. This role is ideal for someone with experience in healthcare marketing, community outreach, home care, senior services, social work, healthcare sales, or business development. Responsibilities Develop and maintain referral relationships with hospitals, rehabilitation facilities, physician offices, senior centers, assisted living communities, social workers, case managers, OLTL Supports Coordinators, Area Agencies on Aging, and other community organizations. Conduct regular in-person outreach visits throughout Montgomery and Chester Counties, with future expansion into surrounding service areas. Represent Happy Seniors Homecare at networking events, health fairs, educational programs, and community events. Identify new referral opportunities and strategic community partnerships. Maintain accurate records of outreach activities, meetings, and referral contacts. Follow up consistently with referral sources and prospective community partners. Collaborate with agency leadership on growth initiatives and community engagement strategies. Submit weekly reports outlining outreach efforts, meetings held, referral activity, and partnership opportunities. Qualifications Experience in healthcare marketing, community outreach, home care, senior services, social work, healthcare sales, business development, or a related field preferred. Strong communication, networking, and relationship-building skills. Self-motivated, organized, and able to work independently. Professional appearance and demeanor. Excellent follow-up and time management skills. Valid driver's license and reliable transportation required. Willingness to travel throughout the assigned service area. Familiarity with Pennsylvania home care services, managed care organizations, or the OLTL Waiver Program is strongly preferred. Performance Expectations Minimum of 10 in-person outreach visits per week. Minimum of 50 outreach touchpoints per week, including in-person visits, phone calls, emails, and follow-up activities. Development of new referral relationships. Active participation in networking and community events. Consistent growth of referral partnerships. Generation of qualified client referrals and admissions.
