Job Description
Position Summary We are seeking a compassionate, organized, and motivated Case Manager to join our team. This position will provide direct client support while assisting with community outreach, group facilitation, administrative tasks, and grant-related program requirements . The ideal candidate is passionate about serving LGBTQ+ communities, behavioral health, and creating meaningful connections to resources and care. Essential Duties & Responsibilities Provide case management services, referrals, advocacy, and resource coordination for clients Support clients with accessing healthcare, behavioral health services, housing, benefits, insurance, and community resources Conduct community outreach and build partnerships with local organizations and providers Facilitate psychoeducational groups, support groups, and wellness programming Perform administrative duties including documentation, scheduling, tracking services, and communication Assist with grant-related requirements, data tracking, reporting, and compliance activities Maintain accurate records and case notes in accordance with organizational policies Participate in team meetings and support program development initiatives Qualifications Experience in case management, behavioral health, social services, outreach, or community-based work preferred Experience working with LGBTQ+ populations strongly preferred Strong organizational, communication, and administrative skills Ability to work independently and manage multiple responsibilities Experience facilitating groups preferred but not required Knowledge of behavioral health systems, Medicaid, or insurance processes is a plus Schedule & Compensation Part-time position with flexible hours Occasional evening/community event availability may be needed Compensation based on experience
