Job Summary A mission-driven nonprofit organization in Santa Clara, CA is seeking a Community Outreach Specialist to support Enhanced Care Management services for adults, caregivers, and families navigating complex health and social service needs. The Community Outreach Specialist will play an important role in outreach, intake, enrollment, referrals, and participant engagement. • Perform telephonic and in-person outreach to engage potential Enhanced Care Management participants and explain available program benefits.
• Meet with participants in community, office, and other approved settings to support intake, enrollment, consent collection, and engagement.
• Use research tools, provider contacts, referral lists, and health plan information to locate and follow up with eligible participants.
• Coordinate referrals to community-based, social support, and health-related resources based on participant needs.
• Maintain accurate documentation, data entry, reporting, and compliance records in accordance with confidentiality and program requirements.
• Participate in community outreach events, presentations, audits, and other program support activities as assigned. Compensation and Benefits

• Compensation:

  $28.00 to $31.00 per hour.

• Work Arrangement:

  Hybrid schedule with 4 days in office and 1 day work from home once training is complete.

• Travel:

  Planned local travel during working hours as needed for in-person outreach and community events.
