Dallas-Fort Worth, TX Trinity Path Homecare is seeking a motivated and relationship-driven Community Outreach & Healthcare Liaison to help grow our non-medical home care services throughout the DFW area. This opportunity is ideal for someone with experience or connections within healthcare, senior care, assisted living, rehab facilities, hospitals, hospice, home health, or community organizations. The ideal candidate enjoys networking, building relationships, and helping families connect with compassionate care and support services. Responsibilities

• Build and maintain professional relationships within the healthcare and senior care community
• Introduce Trinity Path Homecare services to families and professionals
• Identify opportunities where individuals may benefit from in-home support
• Conduct community outreach and networking activities
• Represent Trinity Path Homecare professionally and compassionately
• Communicate referral opportunities and outreach activity with leadership Services We Provide
• Companion care
• Personal care assistance
• Respite care
• Post-hospital recovery support
• Transportation assistance
• Meal preparation
• Medication reminders
• Light housekeeping Qualifications
• Strong communication and interpersonal skills
• Self-motivated and professional
• Healthcare, senior care, outreach, admissions, marketing, or social work experience preferred
• Existing healthcare or community relationships are a plus
• Ability to work independently
