Job Description
EverHope Primary Home Care - The Woodlands, TX EverHope Primary Home Care is a locally owned and operated agency dedicated to helping seniors and individuals maintain their independence through compassionate, non-medical care in the comfort of their own homes. We are seeking a friendly, professional, and self-motivated Community Outreach Representative to help build relationships throughout Montgomery County, Harris County, and surrounding areas. This position is ideal for someone who enjoys networking, building relationships, and independently identifying opportunities to increase EverHope's visibility within the healthcare and senior care community. Schedule Part-time Approximately 12 hours per week Flexible schedule (3 days per week, approximately 4 hours per day) Responsibilities Research and identify potential referral sources, including hospitals, physician offices, rehabilitation facilities, senior living communities, adult day centers, community organizations, and other healthcare-related businesses. Develop and maintain an outreach plan, including identifying new locations to visit each week. Research, identify, and coordinate participation in community events, health fairs, senior expos, networking events, and other outreach opportunities. Visit referral sources and community organizations to introduce EverHope's services and establish professional relationships. Build and maintain relationships with case managers, social workers, discharge planners, physicians, senior living staff, and community leaders. Deliver marketing materials and educational resources to prospective referral partners. Represent EverHope at community events and networking functions. Track outreach activities, contacts made, follow-up opportunities, and event participation. Provide weekly reports detailing visits completed, contacts established, upcoming opportunities, and recommendations for future outreach efforts. Identify new marketing and partnership opportunities that may help grow the agency's presence in the community. Education & Experience High school diploma or GED required. Associate's or Bachelor's degree in Healthcare Administration, Marketing, Communications, Social Work, Public Health, Business, or a related field preferred. 1-2 years experience preferred Previous experience in healthcare, senior services, community outreach, business development, marketing, relationship management, or a related field preferred. Experience working with hospitals, physician offices, rehabilitation facilities, senior living communities, or community organizations is a plus. Preferred Qualities Comfortable networking and meeting new people. Strong communication and interpersonal skills. Excellent organization and follow-up skills. Ability to work independently with minimal supervision. Self-motivated and proactive in identifying new outreach opportunities. Strong research skills with the ability to identify referral sources, community events, and partnership opportunities. Professional appearance and demeanor. Reliable transportation and willingness to travel throughout Montgomery County, Harris County, and surrounding areas. Passion for serving seniors and helping families connect with community resources. Bilingual (Spanish and English) Required Compensation Hourly pay based on experience. Mileage reimbursement.
