Admissions Overview Hampden Hills Post Acute is seeking an experienced, motivated, and resident‑focused Admissions Director to lead our admissions process and serve as the first point of contact for prospective residents, families, referral partners, and community organizations. This role is critical in ensuring a smooth, compassionate, and efficient transition for every resident entering our skilled nursing and post-acute care facility. Key ResponsibilitiesAdmissions & Intake Management Manage the entire admissions process from inquiry to move-in, ensuring accurate documentation and timely communication. Conduct on-site and off-site clinical pre-admission screenings in collaboration with clinical leadership. Coordinate with nursing, therapy, social services, and business office teams to support seamless transitions. Maintain knowledge of eligibility requirements for Medicare, Medicaid, Managed Care, and private insurance. Customer Service & Family Relations Serve as the primary point of contact for families, residents, and referral sources. Provide compassionate guidance during the admissions journey—answering questions, setting expectations, and delivering exceptional customer service. Conduct facility tours and present Hampden Hills Post Acute's services in a professional and engaging manner. Marketing & Outreach Build and maintain strong relationships with hospitals, case managers, physicians, and community partners. Track market trends and competitors to support census development efforts. Represent the facility at community events, hospital meetings, and networking opportunities. Data, Reporting & Compliance Maintain accurate census data, admissions logs, and referral tracking systems. Prepare weekly and monthly admissions reports as required by leadership. Ensure all admissions comply with federal, state, and local regulations. Qualifications 2-3+ years of admissions experience in a skilled nursing facility (SNF), post-acute care, or healthcare setting required. Strong understanding of Medicare/Medicaid, managed care authorization processes, and healthcare eligibility guidelines. Excellent communication, customer service, and interpersonal skills. Proven ability to build relationships with referral partners and drive census growth. Highly organized, detail‑oriented, and able to manage multiple priorities. Ability to work collaboratively with interdisciplinary teams. Valid driver's license and reliable transportation. Preferred Skills Background in marketing, business development, or healthcare administration. Experience with EMR/electronic admissions systems (e.g., PointClickCare).

$55,000.00 - $65,000.00 per year
