Shift:
Tuesday-Saturday 8:00am-4:30pm Participates in marketing to physicians/physician practices to build referral relationships. Supports consumer marketing and public awareness through participation in community events, educational lectures/speaking, and/or other extension of clinical expertise to the community. Maintains productivity standards as defined by the department, through effective time management and clinical efficiency and adjusts work hours to meet the needs of the patients as required. Recommends treatment alternatives for candidates denied admission. Establishes and maintains professional relationships with healthcare individuals and organizations that refer candidates for inpatient rehabilitation. Communicates effectively with referring and supporting healthcare individuals and organizations to ensure timely and clear exchange of necessary information. Coordinates and directs tasks accomplished by personnel supporting this position. Conducts on-site visits with referring healthcare individuals and organizations to assess candidates and review available programs. Participates in practice changes as a result of performance improvement activities. Supports quality standards and initiatives set by the department. Facilitates Payor Authorization and timely patient placement into rehab facility, escalating to leadership as appropriate. Practices principles of employee and patient safety during daily job activities. Facilitates patient's movement through the program by providing proactive planning and creating solutions to ensure seamless care. Documents accurately, completely, and timely using measurable and objective terminology following standards of practice, insurance guidelines and department/hospital policy and procedures. Evaluates, assesses, and communicates patient candidacy for inpatient rehabilitation timely as per department standards. Initiates clinical evaluation of candidates post-referral and makes decisions regarding acceptance or denial based on established criteria. Other duties as assigned.Knowledge, Skills, and Abilities:
- Understands and can communicate effectively with both external and internal stakeholders regarding the continuum of care.
- Must be able to multi-task and effectively manage multiple referral sources.
- Possesses excellent organizational and communication skills, demonstrating a commitment to patient and partners' oriented practice.
- Good computer skills, including proficiency in Word, Excel, Outlook, and basic knowledge of Microsoft Office and Windows.
- Ability to read, write, and speak conversational English effectively.
Education:
- Bachelor's [Required]
Field of Study:
- in Occupational Therapy ,
Physical Therapy or Speech Language Pathology Required Work Experience:
- 2+ year experience in clinical setting [Required]

Licenses and Certifications:
