Organizes information and promotional materials about a school, conducts interviews with students and families, reviews application materials, and works with other school or college administrators to plan admissions goals and identify financial aid resources for admitted students. May travel to meet and recruit students.
Andover Village Rehabilitation and Wellness is a 99 all-private room skilled nursing facility with Joint Commission accreditation, a CMS 5-Star Rating, a ventilator care program, and on-site hemodialysis. We receive referrals from across Northeast Ohio, Eastern Pennsylvania, and many other states. We are seeking an experienced Hospital Liaison to build relationships with hospitals, complete evaluations, support admissions, and help patients transition safely and smoothly into skilled care. Andover Village is seeking a motivated, relationship-driven Hospital Liaison to support referrals and admissions across Northeast Ohio, Eastern Pennsylvania, and beyond. Our advanced clinical programs, private-room model, and ability to accept high-acuity patients make us a preferred referral partner for hospitals in multiple states.
RESPONSIBILITIES
Build and maintain strong relationships with hospitals, case managers, discharge planners, and physicians Conduct hospital visits and clinical evaluations across a multistate territory Respond promptly to referrals, including high-acuity and specialty-care patients Coordinate admissions with the Nursing and Admissions teams Provide tours and assist families in understanding the admissions process Represent Andover Village at hospital meetings, healthcare events, and community outreach activities Support census development and distribute marketing materials as needed
QUALIFICATIONS
Experience as a hospital liaison, admissions coordinator, healthcare marketer, or discharge planner preferred Knowledge of skilled nursing, post-acute care, ventilator/respiratory patients, and dialysis referrals helpful Understanding of Medicare, Medicaid, and insurance authorization processes Strong communication, customer service, and relationship-building skills Ability to work independently, manage a broad territory, and travel regularly Clinical background (LPN, RN, STNA) is a plus but not required
