Hospital Liaison The Heights Rehabilitation and Healthcare Center Broadview Heights, Ohio The Heights Rehabilitation and Healthcare Center is seeking an outgoing, relationship-driven Hospital Liaison to join our team. This position plays an important role in building strong partnerships with hospitals, physicians, case managers, social workers, and other referral sources while helping patients and families navigate the transition to skilled nursing and rehabilitation services. The ideal candidate is a confident communicator who understands the healthcare referral process, enjoys developing professional relationships, and is passionate about connecting individuals with high-quality care. Key Responsibilities Develop and maintain strong relationships with hospital discharge planners, case managers, social workers, physicians, and other referral partners Serve as a knowledgeable and responsive representative of The Heights within the healthcare community Identify and evaluate potential admissions in collaboration with the facility's admissions and clinical teams Educate patients, families, and referral sources about available rehabilitation, nursing, and specialty services Conduct regular hospital and referral-source visits to promote facility services Provide timely follow-up on referrals and assist with coordinating smooth transitions of care Participate in networking events, community outreach, and business-development activities Track referral activity, market trends, and opportunities for census growth Maintain professionalism, confidentiality, and compliance with all applicable healthcare regulations Qualifications Previous experience in healthcare marketing, admissions, business development, case management, or a related role preferred LPN or RN licensure preferred, but not required Knowledge of skilled nursing, rehabilitation, hospital discharge planning, or post-acute care is highly desirable Excellent communication, relationship-building, and presentation skills Self-motivated, organized, and comfortable working independently in the community Ability to manage multiple referrals and priorities with timely follow-through Reliable transportation and willingness to travel throughout the local market Why Join The Heights?
