Hospital Liaison Full-Time | Skilled Nursing & Post-Acute Care Are you a relationship-driven healthcare professional with experience building partnerships in the acute care setting? We are seeking an experienced Hospital Liaison to strengthen referral relationships, grow admissions, and serve as a trusted resource for hospital discharge planners, case managers, and physicians. This is an exciting opportunity to join a growing organization that values collaboration, integrity, and exceptional patient outcomes. What You'll Do Develop and maintain strong relationships with hospitals, case managers, discharge planners, physicians, and other referral sources. Generate referrals and increase admissions through strategic outreach and consistent relationship management. Conduct daily hospital rounds to identify appropriate post-acute care candidates. Educate referral sources on available skilled nursing services and clinical capabilities. Collaborate with facility leadership and admissions teams to ensure smooth patient transitions. Track referral activity, admissions, and market trends. Represent the organization professionally within the healthcare community. Qualifications Experience in healthcare marketing, business development, admissions, or hospital liaison work preferred. Skilled Nursing Facility (SNF), post-acute, home health, hospice, LTACH, or hospital experience strongly preferred. Knowledge of Medicare, managed care, and post-acute discharge planning. Excellent communication, networking, and relationship-building skills. Self-motivated with the ability to work independently and achieve growth goals. Reliable transportation and willingness to travel locally.
