We are seeking an Admissions Liaison to join our team at Lock Haven Rehabilitation & Senior Living, a renowned provider of comprehensive post-acute and long-term care services. As an Admissions Liaison, you will play a vital role in ensuring a seamless transition for patients from acute care settings to our rehabilitation and long-term care programs.
Responsibilities:
Collaborate with hospital discharge planners, physicians, and other healthcare professionals to facilitate admissions and transitions for patients Conduct thorough assessments of patients' needs and develop personalized care plans to ensure optimal outcomes Coordinate with interdisciplinary teams to ensure continuity of care and address patient concerns Educate patients and their families on our programs and services, promoting informed decision-making and empowering them to take an active role in their care Utilize effective communication and problem-solving skills to resolve complex patient situations Maintain accurate and detailed records of patient referrals, admissions, and discharges Participate in ongoing quality improvement initiatives to enhance patient satisfaction and outcomes
Requirements:
High school diploma or equivalent required; degree in a related field (e.g., healthcare, social work) preferred 1+ years of experience Strong communication, organizational, and problem-solving skills Ability to work effectively in a fast-paced environment and adapt to changing priorities Compassionate and patient-centered approach to care
