Job Description

The Lineage Care Group - including The GreenFields and Schofield Care - is seeking a dedicated and clinically knowledgeable Admissions Screener to join our team. This role is essential in evaluating potential residents, coordinating referrals, and strengthening relationships with hospitals and community partners throughout Western New York. Position Summary:
The ideal candidate will combine strong clinical expertise with excellent communication and organizational skills to support admissions growth and ensure a seamless transition for incoming residents. Conduct comprehensive on-site and remote screenings of prospective residents at hospitals, rehabilitation centers, and other referral sources to determine clinical appropriateness for admission. Complete detailed pre-screen assessments and enter accurate clinical, demographic, and payer information into electronic systems in a timely manner. Review medical records, physician notes, therapy reports, and medication lists to evaluate care needs and ensure alignment with facility capabilities. Research and verify discharge plans for short-term and long-term residents, ensuring safe and appropriate transitions into the facility. Collaborate closely with the Director of Admissions, nursing leadership, and interdisciplinary team members to review and approve admission candidates. Communicate clearly and compassionately with patients, families, and responsible parties regarding the admissions process, levels of care, and expectations. Interpret and explain Medicare, Medicaid, managed care, and private insurance guidelines, including coverage criteria and authorization requirements. Build, develop, and maintain strong working relationships with hospital discharge planners, case managers, social workers, and referral coordinators to drive referral volume. Serve as a visible and responsive liaison representing The GreenFields and Schofield Care within assigned hospitals and community facilities to enhance reputation and partnership engagement. Track, prioritize, and manage multiple referrals simultaneously, ensuring timely follow-up and communication to referral sources. Participate in daily census and admissions meetings, providing updates on referral status, pipeline activity, and potential barriers to admission. Assist with marketing and outreach efforts by distributing materials and promoting services within the community and referral network. Ensure compliance with all regulatory requirements, organizational policies, and confidentiality standards (HIPAA) throughout the admissions process. Attend trainings, workshops, and in-services to maintain current knowledge of clinical practices, reimbursement changes, and industry trends. Support cross-training initiatives and assist team members as needed to ensure coverage and continuity across all referral sources. Perform additional duties and special projects as assigned. srhp
Qualifications:
Education and Training:
Minimum of an associate degree in a healthcare-related subject required.
Experience:
Minimum of one year of experience in a healthcare-related field. Long-term care and/or hospital experience preferred.
Character:
The individual must possess good moral character, patience and compassion for the chronically ill, disabled, and elderly.
