Clinical Liaison Location:
Aiken Rehabilitation and Care Center-Aiken, SC Compensation:
$80,000-85,000 DOE What You'll Do As the Clinical Liaison for Aiken Rehabilitation and Care Center, you'll serve as the primary relationship builder between local hospitals, physicians, healthcare providers, and our skilled nursing facility. You'll play a vital role in increasing admissions by developing strong referral partnerships and ensuring a seamless transition for patients into our care. Responsibilities Build and maintain relationships with hospital discharge planners, case managers, social workers, physicians, and other referral sources. Increase admissions and skilled census for Aiken Rehabilitation and Care Center. Grow Medicare and managed care referrals. Conduct clinical and financial reviews to determine admission eligibility. Coordinate communication between referral sources, Central Admissions, and the interdisciplinary team. Meet with prospective residents and families to discuss services and provide facility tours. Represent Aiken Rehabilitation and Care Center within the local healthcare community through networking and community outreach. Develop new referral partnerships while maintaining existing referral relationships. Collaborate with facility leadership to develop and execute strategic marketing initiatives. Track referral activity and market trends to identify growth opportunities. Qualifications Required Minimum of 5 years of post-acute care business development experience. Skilled Nursing Facility (SNF) marketing or Clinical Liaison experience. Experience building relationships with hospital referral sources. Ability to complete clinical and financial admission assessments. Excellent communication, organization, and relationship-building skills. Valid driver's license with the ability to travel locally for meetings and referral visits. Preferred Experience with Assisted Living (AL) and/or Independent Living (IL) business development. Knowledge of Medicare, managed care, and post-acute reimbursement. Existing relationships within the local healthcare community. Why Join Aiken Rehabilitation and Care Center?
