Provide counseling and direct social work services to assist with meeting psychosocial needs of patients and families. Promote achievement of patient/family optimal level of physical and social functioning. Facilitates and coordinates development of discharge plan of care for patients as needed. Responsible for developing the bereavement program for patients and families while under the care of hospice and the 13 months following. Helps to determine bereavement needs of patients and families in order to provide resources and community referrals if needed. Develops and maintains a volunteer program including volunteer supervision, education and recruitment. Oversees volunteer program activities and ensures the program meets the required regulatory compliance. Qualifications for the job... Bachelor's degree in Social Work and currently licensed by State of Alabama. Valid Alabama driver's license, proof of current automobile liability insurance and reliable transportation required to carry out patient assignments.
