We're looking for a compassionate PRN Medical Social Worker (MSW) to join our growing Phoenix hospice team. This is a great opportunity for an experienced social worker seeking a flexible schedule while making a meaningful difference in the lives of patients and families. Develop individualized plans of care addressing emotional, social, financial, and resource needs. Provide counseling, crisis intervention, grief support, and emotional guidance. Assist patients and families with advance care planning and end-of-life decision-making. Connect patients and caregivers with appropriate community resources. Collaborate with nurses, physicians, chaplains, aides, and other members of the interdisciplinary hospice team. Maintain accurate and timely documentation in accordance with agency, state, and federal requirements. Participate in care conferences and interdisciplinary team meetings as needed. Qualifications Master's Degree in Social Work (MSW) from an accredited program. Previous hospice experience preferred . Strong communication, counseling, and interpersonal skills. Ability to work independently in the field while collaborating with the hospice team. Valid Arizona driver's license, reliable transportation, and current auto insurance. Why Dependable Hospice?
