In this vital role, you will provide comprehensive social work services to diverse patient populations, ensuring holistic care that addresses emotional, social, and practical needs. Your expertise will support patients and their families through complex medical journeys, including end-of-life care, recovery, and community reintegration. Duties Conduct thorough patient assessments to evaluate social, emotional, and environmental factors impacting health and recovery. Develop personalized care plans aligned with clinical guidelines, patient preferences, and community resources. Provide clinical counseling, including behavioral therapy, motivational interviewing, and crisis intervention for individuals and groups. Coordinate discharge planning by connecting patients with community resources, home care services, hospice & palliative medicine programs, and support networks. Maintain accurate medical documentation in electronic medical records (EMR) systems while adhering to HIPAA regulations and social work practice standards. Collaborate with healthcare professionals across disciplines to ensure comprehensive treatment approaches and optimal patient outcomes. Engage in clinical research activities related to social work practices or specific patient populations such as geriatrics or pediatrics. Support patients from diverse cultural backgrounds by applying culturally competent social work practices and understanding specific needs related to disabilities or developmental conditions like autism or dementia. Experience Valid licensure as a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW). Proven experience in medical social work within hospital, inpatient, outpatient, nursing home, or hospice settings. Familiarity with medical terminology, CPT coding, ICD-9/10 coding systems for documentation purposes. Experience working with individuals across the lifespan—including children, seniors, or individuals with disabilities—especially in settings like assisted living or post-acute care environments. Strong knowledge of behavioral health interventions such as cognitive behavioral therapy (CBT), applied behavior analysis (ABA), and mental health counseling techniques. Ability to manage crisis situations effectively through crisis management skills and emergency response protocols. Proficiency in using EMR systems for documentation and data collection while maintaining compliance with HIPAA standards. Demonstrated understanding of Medicare regulations, discharge planning procedures, and community resource coordination strategies.
