Contract / Travel Assignment Job Description ATC Healthcare of Arizona is seeking a qualified Social Worker for a 13 week contract assignment at a large hospital in West Prescott, AZ . Key Responsibilities Assess patients' psycho-social needs and develop appropriate care or discharge plans Provide counseling, crisis intervention, and resource coordination Collaborate with interdisciplinary healthcare teams to ensure continuity of care Advocate for patients and assist with referrals to community resources Maintain accurate documentation in accordance with facility, state, and federal guidelines Support patients and families in navigating healthcare, social, and financial challenges Qualifications Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited program Active Arizona Social Work license (or ability to obtain) Minimum of 1 year of social work experience (healthcare or facility‑based experience preferred) Strong communication, organizational, and advocacy skills Ability to adapt quickly to new environments and policies Why ATC Healthcare of Arizona?
