Duties:

(include but not limited to) Complete intakes, facility tours, initial evaluations, reassessments, quarterly assessments, home assessments/visits, and flowsheets. Provide weekly support group counseling, case management, and discharge planning. Work closely with the LCSW providing counseling and to participants with mental health needs and more.

Required Language:

Fluent in English/Mandarin
